MEMBER ADVOCATE DISTRICT MEETING SCHEDULE

 

Representatives of the Fund Office will be out to discuss the impact the Affordable Care Act (Obama Care) has had on the Health & Welfare Fund and to answer your questions about your Health & Welfare and Pension benefits.

All of the following meetings will be held on Saturday at 9:00 in the District Hall:   

District 2 - Joliet - September 7th

District 6 - Lakeville Indiana - September 21st

District 7 - Merrillville Indiana - RESCHEDULED TO OCTOBER 26th

District 5 - Utica - October 12th

District 8 - Rock Island - October 19th

District 3 - Lakemoor - November 2nd

District 4 - Rockford - November 9th

District 1 - Countryside - November 16th

The Midwest Benefit Pharmacy is pleased to announce that our equipment and software will be upgraded in order to serve our members more efficiently. The new equipment and software contains enhanced safety features, a new toll free automated phone refill system, and a website based refill system. In order to accomplish this upgrade, it will be necessary to close the Pharmacy at 5:00 pm on Friday August 23rd and remain closed Saturday August 24th. The Pharmacy will re-open at 8:00 am on Monday August 26th.

Please remember this when ordering refills and allow extra time for the Pharmacy to process your order. As a reminder, you can refill a 90 day prescription as soon as you are down to 30 days of medication left. On a 30 day prescription you can reorder when you are down to 13 days of medication remaining.

DOMINICK’S is once again offering discounted Flu, Pneumonia, Shingles and Tetanus shots to Local 150 members and families.

Flu Shots - $17.00
Pneumonia Shots - $80.00
Shingles Shots - $180.00
Tetanus Shots - $55.00

Simply download and print a voucher and visit your local Dominick’s Pharmacy (no appointment is necessary). All of these vaccinations are reimbursable expenses under you and your spouse’s $350 annual wellness benefit.  If you and/or your spouse have already exhausted your wellness benefit or if your dependents need one of these vaccinations, you can use your $1,500 Annual Family Supplemental Benefit.  CLICK HERE to download a Family Supplemental Benefit Claim form.  Complete the form and submit it to the Fund Office along with your paid receipt.

Adult Dependents NOw COVERED to age 26

 

Effective January 1, 2011, the Health & Welfare Plan covers dependent children up to age 26, regardless of circumstances such as being a full-time student, being married, or place of residence.  As long as you remain eligible under the Plan, your adult dependent is covered up until their 26th birthday.  The Plan will exclude any child age 19 or older who is eligible for other group coverage through his or her employer, or through his or her spouses employer, regardless of wether the child enrolls in such coverage. (This provision will not apply starting January 1, 2014.)

Need medical care while traveling?

If you need emergency medical care anywhere, go to the nearest facility for treatment. If you need non-emergency medical care while outside the Illinois area, call BCBS at 800.810.2583 to find a network provider near you or log on to BCBS.


Do you work out of the areA?

If you work outside of Local 150’s jurisdiction for another local, you may transfer contributions made to another local union pension plan to the MOE Pension Plan provided the other pension plan is a plan reciprocal to Local 150. Call your District Office or 708.579.6620 or click here to request a Transfer Authorization Form. After the Fund Office receives your Transfer Authorization Form, future contributions will be transferred from the other plan to the MOE Pension Plan. You can also transfer up to six months of contributions going back before you submitted the Transfer Authorization Form.


if you're retired and return to work, DID YOU NOTIFY THE FUND OFFICE?

If you’re retired, receiving benefits and you return to work, you must inform the Trustees in writing within 30 days of starting work. Certain work after retirement is considered disqualifying and can result in a suspension of your pension benefits. Before you reach normal retirement age, your benefits will be suspended for any month you work in disqualifying employment. After you reach normal retirement age, your benefits will be suspended in any month in which you work at least 41 hours in disqualifying employment.
