District 5 Open Enrollment

The purpose of the appointments at this event is for you and your spouse to meet one-on-one with a Fund Office navigator with the goal of selecting a health plan option for coverage beginning April 1, 2022 through March 31, 2023.  These appointments are limited to 30 minutes so please come prepared with your questions and review the open enrollment material prior to your scheduled appointment to optimize the allowed time.

REMINDER: COVID-19 PROTOCOLS

Please adhere to the following protocols:

• If you are experiencing any COVID-19 symptoms (fever, cough, shortness of breath, chills, headache, muscle pain, sore throat, loss of taste and/or smell), please cancel your appointment and re-schedule or request for a phone consultation
• You can attend your appointment with one additional companion
• You and your companion will be required to have a temperature scan upon entry to your appointment
• Due to the nature of your appointment, alternative childcare arrangement should be made
• You and your companion must wear a mask for the entire duration of your appointment
• Only coffee and/or water will be allowed during your appointment
