REMINDER….Don’t forget to submit your Family Supplemental Benefit claim forms, along with your itemized bill or your Explanation of Benefits (EOB) that relates to the claim and your paid receipt. Your FSB claim must be received by the Fund Office within one year (12) months of the date the expense is incurred. Your FSB will reset at the start of each Plan Year:
Active Welfare Fund – Plan Year is from April 1 – March 31
Retiree Welfare Plan – Plan Year is from January 1 – December 31
