Information on COVID-19 Vaccinations

If you meet your current state’s guidelines to receive a COVID-19 vaccination, below are resources to assist you in making an appointment:

Illinois: https://www.dph.illinois.gov/covid19/vaccine-faq

Eligibility: Currently it appears Illinois is use a Phase system for eligibility for the vaccine, based on exposure risks and vulnerable population.

No internet access: Call (833)621-1284 7 days a week/6am-midnight

Indiana: https://www.coronavirus.in.gov/vaccine/index.htm

Eligibility: Restricted to Indiana residents age 50 or over, or specific groups of patients at high risk. Call 211 if no internet access.

Hoosiers age 50 and older can schedule a vaccination at the link in the red bar above or by calling 211 if they do not have access to a computer or need assistance registering.

Eligibility: Currently it appears that Wisconsin is using a Phase in system for vaccine eligibility. They have a Vaccine Registry set up(see below)

The vaccine registry(link is external) is now available. If you live in an area where the vaccine registry is available and have questions or need help registering, call (844) 684-1064 (toll-free) between 7 a.m. and 7 p.m.

Eligibility: Currently it appears Iowa is using a Phase in system for vaccine eligibility.

If you have general questions about the vaccine, call 211.  Resource specialists are available 24 hours a day to answer basic questions. Check with your health care provider with questions specific to your health situation.

Eligibility: Currently it appears Michigan is using a Phase in system for vaccine eligibility.

  • Residents who don’t have access to the internet or who need assistance navigating the vaccine scheduling process can call the COVID-19 Hotline at 888-535-6136 (press 1) Monday through Friday from 8 a.m. to 5 p.m. Saturday and Sunday 8 a.m. to 1 p.m.
